These awards are the next step for the Be the Change for Kids initiative. It highlights successful programs that are making a difference – programs that set the stage for our children’s future successes – in college, career, community and as citizens.

- Can be sustained over time.
- Have the potential to be adapted for other grade levels.
- Relate to or prepare students for science, technology, engineering, and mathematics careers.
- Lead to be better opportunities and greater success for students.
Each school district must complete and submit a nomination form online. Please note, this online form cannot be saved to your computer. Therefore, we are providing the questions and information in this PDF to assist you in preparing your answers in advance. If you need to send supporting documents and materials but are not able to submit them online, you may send them via mail to New York State School Boards Association in care of Be the Change for Kids, 24 Century Hill Drive, Suite 200, Latham, NY 12110-2125 or emailed to email@example.com.
